What You Should Know About Your First Visit With Me

I am so glad that you have decided to schedule an appointment with me. I am looking forward to helping you find freedom from pain, regain your mobility or work on resolving whatever your condition may be. Today, I feel honored to be able to treat the most fascinating, complicated, and grateful patients in the world.

There are a few things that we will need to have completed before I can begin treatment of your condition(s). Below is a list of those items. If you feel lost, or have a question, you can either visit our Frequently Asked Questions page or give me a call at (949) 226-9681.

Step 1: Initial Phone Consultation & Assessment

Fill Out and Submit the Intake Form Below

Please begin by clicking the red button below, then complete & submit my HIPAA compliant medical intake history form.

Once the form is submitted, I will review your information and the give you call to discuss your condition in more detail. I can also answer any questions you may have. If I feel we are a good match, I will schedule your first in-office assessment.

Initial physical therapy assessments (evaluation & treatment) are $175.

Medical History Form
  • Begin by completing & submiting my HIPAA-Compliant "Medical History Form."
  • Once the form is submitted, I will review the form, give you call to discuss your condition in more detail and answer any questions you may have. If I feel we are a good match, I will schedule your first in office assessment. Initial physical therapy assessments (evaluation & treatment) are $160.

Step 2: Items to complete PRIOR to your first visit

I know your time is valuable. To help save us both time and get the most out of your first appointment, please read through and complete this list of items below:

  • If you have not already filled out your medical history intake form, complete & submit my new patient questionnaire HERE (this form is secure and HIPAA compliant).
  • Print out and sign the following PDF forms. Please bring the signed copies with you OR fax you completed forms to (949) 220-7891.
  • Patient Arbitration Agreement from.
  • Informed Consent form.
  • HIPAA "Your Health and Privacy Rights" form.
  • Complete & submit my patient intake form HERE (this form is secure and HIPAA compliant).
  • Print out and sign the following PDF forms. Please bring the signed copies with you.

How In Office Assessment Works

This video demonstrates the process of assessment on an actual patient who came in with neck pain.

Appointment Cancelation Policy

Failure to show up for an appointment, and not calling to cancel the visit in advance, may result in a fee. Canceling scheduled appointments is disruptive to the therapist’s schedule. If an emergency prevents you from attending, try to provide adequate notice.

Getting the most out of your physical therapy sessions

  • Have all the forms listed above filled out and arrive for treatment sessions at the scheduled time or a few minutes early so you are prepared. Late arrival may affect your one-on-one time with the therapist.
  • Actively participate in the discussion with your therapist to help determine what the frequency of your visits should be. Work in partnership with your physical therapist to help achieve your treatment goals.
  • You will get out of therapy what you put into it. Sufficient effort, as agreed between you and the physical therapist, is necessary to maximize benefit from each treatment session.
  • Follow the home program as instructed by the physical therapist. Your ongoing performance and commitment to the home program is essential to your recovery.
  • If the instructions are unclear, ask for clarification. Only perform exercises at the therapist-specified repetition, frequency, and resistance. More is not always better and may cause injury!
  • If you plan to discontinue therapy or change the frequency of treatment due to personal or financial reasons, be sure to discuss this with your physical therapist in advance. See above for cancelation policy.

What can I expect from my first visit?

After your forms are complete, bring doctors prescription (if you have one), insurance card if you have a PPO, and any tests or information you would like to share or review. Then a physical therapy evaluation and treatment will be done. A discussion of findings, treatment, goals, home program, what to expect, appointments scheduled and payment will be collected.

Can I complete my paper/computer work before arriving?

Yes, please see the STEP 2 section above, then print and complete the necessary forms before our first visit. This gives us more time for treatment.
